Incorrect · Treating hypoglycemia first Medium

A resident with type 1 diabetes in a long-term care facility is sweaty and confused before lunch. Fingerstick glucose is 52 mg/dL. The resident is awake and able to swallow. What should the practical nurse do first?

A. Notify the provider of the glucose result (your answer)

Reporting matters, but treating the low comes first. Waiting lets the glucose keep falling.

B. Give 15 g of fast-acting carbohydrate ✓ correct

The resident can swallow, so treat now with 15 g of fast-acting carbohydrate, recheck in 15 minutes, then report.

What you get

Study smarter, not just more

Never the same question twice

Every test pulls questions you haven't seen. When you retake what you missed, you get brand-new questions on the same idea, so you learn the concept instead of memorizing an answer.

Every answer explained

After each test, see why the right answer is right and why every wrong option is tempting. That's where the learning happens.

A study guide from your mistakes

Finish a test and get a personal study guide built from the questions you missed: the core concept, key points, memory aids, and questions to check yourself.

A plan that paces you

Set your exam date and a daily goal. Your plan tells you what to do next: retake missed concepts, focus on a weak category, or take a mixed test.

Built on the 2026 test plan

Questions follow the 2026 NCLEX-PN test plan and are weighted across the eight Client Needs categories, like the real exam.

Written for where PNs work

Many questions are set in long-term care and skilled nursing, with the conditions you'll see there: diabetes, trach and ventilator care, wounds, dementia, and more.

How it works

Practice, understand, repeat

  1. 1

    Take a practice test

Pick 10, 25, or 50 questions. One at a time, like the real exam, and answers are final once you submit.

  2. 2

    Learn why

See your score, then review every question: what you chose, the right answer, and the reasoning behind each option.

  3. 3

    Close the gaps

Read your study guide, then retake what you missed with rewritten questions until those concepts move to Mastered.

Matches the exam

Every Client Needs category, in the right proportions

Each practice test spreads its questions across the eight categories of the 2026 NCLEX-PN test plan, weighted the way the real exam is. Your progress page shows your accuracy in each one, so you know exactly where to focus.

Questions

Frequently asked

Are these real NCLEX questions?

No. They are original practice questions written to the 2026 NCLEX-PN test plan. Real NCLEX items are confidential and are never used.

Will I ever see the same question twice?

No. Each test only uses questions you haven't seen yet. When you retake missed concepts, you get new questions on the same idea, so you can't pass on memory alone.

How are the questions written?

Questions are written with the help of AI, following the 2026 NCLEX-PN test plan, the LPN/VN scope of practice, and standard item-writing rules. Each comes with an explanation for every answer choice.

How many questions should I do each day?

Your study plan suggests a daily goal based on how far away your exam is, about 50 questions a day for most of your prep and a light review the day before. You can set your own goal anytime.

Is it only for the NCLEX-PN?

Charted Prep starts with the NCLEX-PN. More exams are on the way.
